A Public Service Commission investigation into South Africa's State IT Agency (Sita) found that governance instability originated above board level — with repeated ministerial changes, ad hoc political interventions, and leadership churn between 2020 and 2025 being the primary root cause of the agency's dysfunction. The report identifies 'sustained leadership instability' as the first corrective responsibility of the executive authority (the minister), not Sita itself. Key incidents include a 2023 ministerial dismissal of board members that was overturned by courts, salary approval failures, and a task team chair installed as acting MD despite conflict-of-interest concerns. The investigation was itself hampered by fragmented record-keeping and the absence of independent digital forensics.
