The Meyer/Williamson matrix is a framework cataloging potential causes of accidents in outdoor activities, organized into three categories: unsafe conditions (weather, equipment, falling objects, etc.), unsafe acts (inadequate protection, supervision, instruction, etc.), and errors in judgment (fatigue, miscommunication, distraction, etc.). Originally from presentations by Dan Meyer and Jed Williamson, this is a stable HTML reproduction of the matrix for easy reference, as existing PDF versions tend to suffer from link rot.
