Two real examples are shown: a travel-concierge agent where mid-conversation revisions were silently ignored (21% failure rate reduced to 5% with a 3-sentence prompt fix), and a bug-triage assistant where tool-usage reporting went from 0% to 96% compliance. The skill works both in dev (with synthesized traces) and against production OpenTelemetry traces, with the same raters grading both.
